As an LVN, you will play a vital role in delivering compassionate nursing services, performing assessments, administering medications and treatments, and ensuring the highest standards of patient safety and confidentiality. Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems. Monitor patients' conditions closely, including airway management, ventilator support, pain management, and post-operative care. Maintain accurate medical documentation in compliance with HIPAA policies while ensuring confidentiality of all patient information. Educate patients and their families on health coaching topics such as medication administration, nutrition, discharge planning, and disease management strategies. Experience Private Duty Nursing monitor for 8 hour , 3x a week or more. Strong understanding of long-term care regulations alongside skills in case management and discharge planning to ensure seamless patient transitions across settings.
