Licensed Vocational Nurse (LVN) -Full-Time PM Shift Facility:
Oakwood Gardens Care Center Shift:
PM Shift Salary Range:
$28.00 - $30.00 per hour (BOE) General Purpose The Licensed Vocational Nurse (LVN) provides direct nursing care to residents and supervises the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs). Care is delivered in accordance with current federal, state, and local regulations, facility policies, and under the direction of the Director of Nursing (DON) or Nurse Supervisor to ensure the highest quality of resident care. Essential Duties & Responsibilities- Provide direct nursing care in accordance with resident assessments and care plans
- Supervise and support CNAs to ensure quality care delivery
- Administer medications and treatments as ordered by physicians
- Accurately document nursing care, medication administration, and resident status
- Monitor residents for changes in condition and report findings promptly
- Participate in admissions, transfers, and discharges as assigned
- Ensure compliance with infection control, safety, and sanitation standards
- Communicate effectively with residents, families, physicians, and interdisciplinary team members
- Assist with restorative and rehabilitative nursing programs
- Respond appropriately to emergencies and resident needs
- Maintain resident confidentiality and comply with HIPAA regulations Supervisory Responsibilities
- Provides direct supervision to CNAs during assigned shift
- Ensures staff adhere to policies, procedures, and scope of practice Qualifications Education & Experience
- Graduate of an approved LVN/LPN program
- Long-term care or skilled nursing experience preferred Licenses & Certifications
- Current, active, and unencumbered LVN license in the State of California
- Current CPR certification
- Knowledge of PCC preferred Skills & Abilities
- Strong clinical, communication, and leadership skills
- Ability to work independently and as part of a team
- Ability to handle fast-paced and emotionally demanding environments
- Professional, compassionate, and resident-centered approach to care Physical Demands
- Ability to stand, walk, bend, and move throughout the shift
- Ability to lift, push, pull, and assist residents (up to 25 lbs routinely; more with assistance)
- Ability to respond to emergencies, including resident evacuation if needed Work Environment
- Skilled nursing facility environment
- Exposure to residents, visitors, and healthcare equipment
- Noise level generally low to moderate
