Job Overview We are seeking a dedicated and compassionate Per Diem Licensed Vocational Nurse (LVN) to join our healthcare team. This role offers flexible scheduling to provide vital nursing care across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. As a Per Diem LVN, you will play a crucial part in delivering high-quality patient care, supporting clinical teams, and ensuring compliance with healthcare regulations. Your expertise will help improve patient outcomes and enhance the overall quality of life for diverse populations, including geriatric patients, individuals with developmental disabilities, and those requiring specialized memory or dementia care. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans. Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA and confidentiality policies. Assist with complex procedures such as IV infusion, catheterization, tube feeding, and ventilator management. Document all patient interactions and clinical activities using Electronic Health Record (EHR) systems like Epic or Cerner with precision and timeliness. Provide wound care, infection control measures, and monitor patients for acute pain management or signs of deterioration. Support triage activities by evaluating patient needs in urgent or emergency situations and coordinating appropriate interventions. Collaborate effectively with multidisciplinary teams to ensure seamless continuity of care across medical-surgical units, geriatrics, pediatrics, or specialty services such as hospice or ICU. Qualifications Valid LVN license in the state of employment with current CPR/BLS certification. Proven experience working in diverse healthcare settings such as hospitals (including Level I Trauma Centers), nursing homes, skilled nursing facilities, or home health environments. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks; familiarity with

ICD-9/ICD-10

coding is a plus. Strong knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to adult and geriatric patient populations. Experience with advanced nursing skills including IV infusion therapy, sterile technique, airway management, and medication administration. Ability to work independently in a fast-paced environment while maintaining attention to detail and clinical accuracy. Excellent communication skills for caregiver communication, client education, and team collaboration; experience working with individuals with disabilities or dementia is highly desirable.
