Riverside Heights Healthcare Center is currently searching for an experienced MDS Coordinator. Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN) specializing in Minimum Data Set (MDS) assessments! In this vital role, you will be at the forefront of patient care documentation, ensuring compliance with healthcare regulations while delivering compassionate, high-quality nursing services. Duties Conduct comprehensive MDS assessments to evaluate residents' clinical needs and functional status, ensuring accuracy and completeness in documentation. Review and analyze medical records, treatment plans, and clinical data to support precise coding and billing processes aligned with Medicare and Medicaid regulations. Collaborate with interdisciplinary teams to develop individualized care plans based on assessment findings, promoting optimal patient outcomes. Ensure compliance with all federal and state healthcare regulations, including CMS guidelines, HIPAA privacy standards, and long-term care policies. Utilize our EMR system, Point Click Care, to document assessments, update patient records, and facilitate seamless information flow. Participate in utilization management activities by reviewing medical necessity for services and supporting documentation review for accuracy. Assist in discharge planning and coordinate with case management teams to facilitate smooth transitions of care within the facility or to external providers. Experience Prior MDS experience in SNF setting with a strong understanding of long-term care regulations and Medicare requirements. Preferred hands-on experience with Point Click Care; proficiency in electronic health records management is essential. Knowledge of health regulation policies including CMS guidelines, HIPAA compliance, and state healthcare regulations governing medical documentation. Strong physiology and anatomy knowledge combined with excellent critical thinking skills to interpret clinical data accurately. Experience in discharge planning, utilization review, case management, and medical management within hospital or outpatient settings is highly desirable.
