Superior Hospice Invites you to Join Our Team! We are seeking a compassionate, organized, and clinically strong Licensed Vocational Nurse (LVN) to join our hospice team in a primarily office-based clinical support role . The LVN will support the clinical team through referral intake, telephone triage, chart monitoring, regulatory compliance support, and coordination of patient care, with occasional PRN patient visits as needed.

What You'll Do:

Receive and input hospice referrals from hospitals, facilities, physicians, and community partners. Maintain patient schedules in coordination with interdisciplinary team. Communicate with community partners, physicians, facilities, and hospitals in a timely and professional manner. Serve as a compassionate clinical resource for incoming patient and caregiver calls. Triage symptoms and concerns of patients and determine the appropriate clinical response. Perform administrative duties to support compliance with federal and state hospice regulations, Medicare requirements, and agency policies and procedures. Maintain accurate, organized, and confidential patient records in accordance with regulatory and agency requirements. Provide PRN hospice visits when needed to support the field staff. Assess patients and provide appropriate skilled nursing interventions within LVN scope of practice and hospice protocols. Assist with symptom management and communicate changes in condition to the appropriate RN/medical provider.

What We're Looking For:

LVN with a current, unrestricted California LVN/LPN license . Knowledge of Microsoft Office suite Hospice, home health, palliative care, or related experience preferred. Strong clinical assessment and critical-thinking skills. Excellent telephone communication and triage skills. Ability to review clinical information and determine appropriate course of action. Strong organizational and time-management skills. Comfortable working independently while collaborating closely with an interdisciplinary team. Compassionate, professional, and patient-centered approach. Ability to maintain confidentiality and handle sensitive patient information appropriately. Reliable transportation and ability to perform PRN field visits.

Pay:

From $30.00 per hour Expected hours: 40.0 per week
