Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately while following strict protocols for IV infusion, catheterization, tube feeding, and other treatments. Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures to support physicians' diagnoses. Manage airway devices such as ventilators and provide acute pain management for patients in emergency room, ICU, or trauma center settings. Document all patient interactions thoroughly in compliance with HIPAA regulations and clinical confidentiality policies. Collaborate with interdisciplinary teams on case management plans for complex cases involving dementia care, Alzheimer's care, hospice services, or infectious disease management. Support patient safety initiatives by monitoring for signs of deterioration, triaging urgent needs, and providing first aid or basic life support when necessary. Qualifications Valid LVN license in accordance with state licensing requirements. Proven experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, skilled nursing facilities, or home & community care settings. Strong knowledge of medical terminology, anatomy, physiology, ICD-10/ICD-9 coding systems, and nursing best practices. Experience with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or similar platforms. Demonstrated proficiency in medication administration techniques including injections and venipuncture. Critical care experience preferred—especially ICU or emergency medicine—along with familiarity in acute pain management and infectious disease care. Ability to work effectively with individuals with disabilities or developmental challenges; experience in dementia or memory care is a plus. Excellent communication skills for caregiver communication and client interaction; ability to work collaboratively within multidisciplinary teams. Knowledge of long-term care regulations and geriatric patient needs; experience working with geriatric patients is highly desirable.
