ABOUT THE ROLE

OverSightMD is a healthcare managed care company delivering Enhanced Care Management (ECM) services to high-risk Medi-Cal members across Santa Clara, Santa Cruz, Monterey, San Benito, and Merced Counties. We're hiring immediately for Licensed Vocational Nurses (LVNs), part-time or full-time, to support care coordination for ECM members with complex medical, behavioral health, and social needs. Working under the supervision of our RN Chief Nursing Officer, you'll help members navigate their care, close gaps, and stay connected to the resources they need.

RESPONSIBILITIES

• Support care coordination for ECM members with complex medical, behavioral health, and social needs
• Conduct health screenings and assessments; collect health information to support chronic disease management
• Assist with treatment plans, discharge instructions, and provider recommendations
• Perform medication reconciliation and educate members on medications
• Monitor member symptoms and communicate concerns to the care team
• Coordinate with Lead Care Managers, Community Health Workers, Behavioral Health staff, providers, hospitals, and health plans
• Complete field-based member visits and community outreach as needed
• Maintain accurate, timely clinical documentation

QUALIFICATIONS

• Active, unrestricted California LVN license
• Current BLS/CPR certification
• 1+ years of clinical experience; care coordination or case management experience preferred
• Experience with at-risk or Medi-Cal populations a plus
• Comfortable splitting time between the office, the field, and member visits
• Strong documentation skills and comfort with EHR systems
• Reliable transportation for member visits within the assigned county
• Bilingual preferred — Spanish, Mandarin, or Vietnamese; Spanish-speaking candidates are especially encouraged to apply, as the majority of our members are Spanish speaking

Pay:

$30.00

00 per hour, depending on county

Schedule:

Part-time (25 hrs/week) or full-time (40 hrs/week), flexible days between Monday

• Friday; in-office and field-based, with some members seen at our office locations

Locations:

Multiple openings across Santa Clara, Santa Cruz, Monterey, San Benito, and Merced Counties. Headquartered in Milpitas, with regional offices in Salinas and Merced.
