The Wound Care LVN performs treatments according to physician orders and the established plan of care, monitors healing progress, educates patients and caregivers, and promptly communicates changes in condition to the supervising Registered Nurse and healthcare team. Essential Duties and Responsibilities Perform scheduled home health visits and complete patient assessments within the LVN scope of practice. Provide wound care as ordered, including cleansing, dressing changes, packing, skin protection, and other approved treatments. Assess and document wound location, type, size, drainage, odor, surrounding skin condition, pain, and signs of healing or deterioration. Monitor patients for infection, complications, changes in condition, and treatment effectiveness. Notify the supervising RN, physician, or other authorized clinician of abnormal findings or changes requiring intervention. Follow the individualized plan of care, physician orders, agency procedures, infection-control standards, and applicable regulations. Educate patients and caregivers about wound care, infection prevention, pressure relief, repositioning, nutrition, medication adherence, and when to report concerns. Reinforce teaching initiated by the RN and verify patient or caregiver understanding. Obtain vital signs and perform additional skilled nursing tasks authorized by the plan of care and permitted by state LVN regulations. Maintain accurate, timely, and complete clinical documentation in the agency's electronic health record. Coordinate visit schedules and communicate professionally with patients, caregivers, nurses, providers, pharmacies, and agency staff. Use supplies appropriately, maintain clean technique or sterile technique as ordered, and report supply needs promptly. Participate in case conferences, quality-improvement activities, competency evaluations, and required training. Protect patient privacy and comply with HIPAA requirements. Report suspected abuse, neglect, exploitation, safety hazards, medication concerns, and adverse events according to policy and law. Follow emergency procedures and maintain professional boundaries in the home setting.
