$36.00-$40.00 per hour Join Our Caring Clinical Team Pleasant Hill Post Acute is seeking a compassionate, dependable, and dedicated Licensed Vocational Nurse (LVN) to join our clinical team on a Part-Time / On-Call basis . The ideal candidate is committed to providing safe, high-quality, resident-centered care while working collaboratively with residents, families, physicians, and the interdisciplinary healthcare team. Responsibilities Provide nursing care and services to residents in accordance with established care plans. Administer medications and treatments as ordered. Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals. Complete accurate and timely nursing documentation. Assist with assessments and contribute to individualized care planning. Communicate effectively with residents, families, and members of the healthcare team. Follow infection control, safety, and facility policies and procedures. Promote residents' comfort, dignity, and overall well-being. Qualifications Current and valid California LVN license . Previous skilled nursing or post-acute experience preferred. Strong communication and organizational skills. Dependable, compassionate, and team-oriented.
