The ideal candidate will have strong clinical skills and experience providing nursing care to individuals with developmental disabilities and complex medical needs. The LVN will work closely with the RN, Administrator, QIDP, Direct Care Staff, physicians, pharmacies, and other healthcare professionals to ensure that each individual receives safe, consistent, and person-centered care. This position requires a professional who is dependable, organized, attentive to changes in health status, and committed to protecting the dignity, safety, rights, and quality of life of the individuals served. Responsibilities Provide nursing care and support to individuals with developmental disabilities and varying medical and healthcare needs. Monitor individuals' health conditions and recognize changes in physical, behavioral, or medical status that may require further evaluation. Obtain and monitor vital signs, blood glucose levels, weights, oxygen saturation, and other clinical measurements as ordered or required. Administer medications and treatments accurately and safely in accordance with physician orders, facility policies, and applicable regulations. Perform nursing treatments within the LVN's scope of practice, including wound care, injections, catheter care, skin care, and other prescribed treatments. Assist with management and monitoring of individuals with chronic medical conditions, including diabetes, seizure disorders, hypertension, respiratory conditions, dysphagia, and other complex healthcare needs. Monitor individuals for signs and symptoms of illness, infection, injury, medication reactions, or other changes in condition and promptly report concerns to the RN and appropriate healthcare providers. Maintain accurate and timely nursing documentation, including progress notes, medication records, treatment records, physician orders, incident documentation, and other required records. Review and follow physician orders, nursing care plans, individual health needs, and recommendations from healthcare professionals. Coordinate and assist with physician appointments, laboratory services, specialist visits, hospital follow-up, and other healthcare services. Communicate effectively with the RN, QIDP, Administrator, Direct Care Staff, physicians, pharmacies, family members, and other members of the individual's interdisciplinary team. Provide direction, training, and education to Direct Care Staff regarding individuals' healthcare needs, medication procedures, infection control, emergency procedures, and other nursing-related responsibilities as assigned. Participate in the development, implementation, and monitoring of individualized healthcare plans and support plans. Maintain appropriate infection-control practices, including hand hygiene, use of personal protective equipment, and proper cleaning and disposal procedures. Respond appropriately to medical emergencies and follow the facility's emergency response procedures. Assist with maintaining medical supplies, medication areas, treatment equipment, and emergency medical supplies. Protect confidential health information and maintain compliance with HIPAA and applicable state and federal requirements. Promote individuals' rights, dignity, independence, safety, and quality of life while providing respectful and person-centered care. Participate in staff meetings, healthcare meetings, training, quality assurance activities, and other facility programs as required. Experience & Qualifications Current and active California Licensed Vocational Nurse (LVN) license in good standing. Current CPR/BLS certification or ability to obtain certification as required. Experience working with individuals with developmental disabilities is strongly preferred. Experience in an ICF/DDN, ICF/DDH, developmental disability, skilled nursing, residential care, hospital, or other healthcare setting is preferred. Demonstrated ability to administer medications and treatments safely and accurately. Experience monitoring and documenting individuals with complex medical and behavioral needs. Knowledge of infection-control practices, medication administration, emergency procedures, and basic nursing standards. Experience with electronic or computerized medical documentation systems is preferred. Strong clinical observation, communication, organization, and documentation skills. Ability to recognize changes in an individual's condition and communicate concerns promptly to the appropriate healthcare professional. Ability to work effectively as part of an interdisciplinary team. Must demonstrate professionalism, reliability, compassion, patience, and respect when working with individuals with developmental disabilities. Must be able to maintain confidentiality and comply with all applicable facility policies, regulations, and licensing requirements.
