Job Summary We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team as a Private Duty Visits Nurse. In this vital role, you will provide personalized, high-quality nursing care directly in patients' homes, ensuring their comfort, safety, and well-being. Your expertise will support individuals with diverse health needs, including those with chronic conditions, developmental disabilities, or recovering from illness or injury. Responsibilities Deliver comprehensive nursing assessments and develop individualized care plans tailored to each patient's unique needs. Administer medications, including injections and IV infusions, following strict protocols and documentation standards. Monitor vital signs, perform physical examinations, and evaluate patient responses to treatments to ensure optimal health outcomes. Manage complex medical procedures such as catheter care, tube feeding, wound dressing changes, and ventilator management. Maintain accurate and detailed medical documentation using Electronic Health Records (EHR) systems while adhering to HIPAA confidentiality policies. Communicate effectively with patients, families, and interdisciplinary healthcare teams to coordinate care and provide education on health management strategies. Respond promptly to urgent situations such as acute pain episodes or respiratory distress by utilizing triage skills and emergency protocols. Experience Candidates should possess prior experience working as a Licensed Vocational Nurse in diverse settings such as home care, skilled nursing facilities, hospitals, or assisted living communities. Experience with the following is highly valued: Epic EMR/EHR systems for documentation and patient record management. Critical care environments including ICU or Level I Trauma Centers. Long-term care regulations and dementia or Alzheimer's disease care. Medical-surgical units, pediatrics, geriatrics, or hospice care settings. Skills in IV infusion therapy, medication administration, infection control practices, and patient assessment. Familiarity with
