JOB DESCRIPTION
LVN
CARE COORDINATOR POSITION SUMMARY
Provides case management for a team of clients using nursing theory and process and in accordance with the state Nurse Practice Act under the direction of the RN Supervisor or the Director of Patient Care Services.
QUALIFICATIONS
1. Graduate of an accredited school of Professional nursing. 2. Current license as a Licensed Vocational Nurse in the state(s) of practice. 3. Minimum of two (2) years of nursing experience. Home care experience preferred. 4. Effective written and oral communication skills and good interpersonal skills. 5. Organizational and time management skills. 6. Knowledge of home health regulatory and reimbursement requirements. 7. Current driver's license, safe driving record, and reliable transportation.
JOB RESPONSIBILITIES
1. Reviews client referral information and responds to requests and inquiries as appropriate. a. Evaluates eligibility and appropriateness of client for home care services. b. Determines appropriateness of referral for home care services and responds per agency policy and standard guidelines. 2. Communicates with other disciplines/departments when required. 3. Makes recommendations that reflect consideration of immediate and long-range effects (frequency of visits, additional services, etc.) 4. Provides on-call support for clients per schedule. 5. Schedules in-home supervision of clinical staff. 6. Collaborates with supervisors to identify staffing needs and communicates this information. 7. Coordinates and follow-up orders for Durable Medical Equipment, medical supplies, etc. to ensure timely delivery to the patients. 8. Coordinates and follow-up orders for blood works, X-ray, and other ordered tests for the patients to ensure timely scheduling of the procedure. 9. Follow-up status of patients that were admitted to the hospital or other health care facility to ensure continuity of care upon patient's release from the facility. 10. Participates in all mandatory education per established guidelines. 11. Performs job duties in accordance with agency policies, procedures, and professional and community standards. 12. Maintains confidentiality in all aspects of the job. Does not reveal information from client records to others, except as identified in agency policy. 13. Participates in the development, implementation, and evaluation of the Quality Improvement Program and activities. 14. Performs other job duties as assigned and completes assigned tasks within established guidelines and time frames.

SPECIAL REQUIREMENTS 1.
Must have a car with required insurance coverage and a state driver's license or reliable transportation 2. Computer literate
