Full-Time / Part-Time / Per Diem About Us JCH Medicare Services Hospice Provider is seeking a compassionate, dependable, and dedicated Licensed Vocational Nurse (LVN) to join our hospice care team in Torrance, California. Our mission is to provide compassionate, patient-centered hospice care that promotes comfort, dignity, and quality of life for patients and their families. Position Summary The Hospice LVN provides skilled nursing services to hospice patients under the direction and supervision of a Registered Nurse and in accordance with the patient's established plan of care. This position is ideal for an LVN who is compassionate, organized, dependable, and comfortable providing care to patients in their homes and other care settings. Key Responsibilities Provide skilled nursing care according to the established hospice plan of care and within the LVN scope of practice. Monitor patients' conditions, symptoms, pain levels, and responses to treatment. Report changes in patient condition promptly to the supervising RN and appropriate members of the hospice team. Administer medications and treatments as ordered and permitted within the LVN scope of practice. Assist with pain and symptom management and provide comfort-focused care. Educate patients, families, and caregivers regarding medications, treatments, safety, and hospice care. Communicate and coordinate with RNs, physicians, home health aides, social workers, chaplains, and other members of the interdisciplinary team. Complete accurate and timely clinical documentation and patient visit notes. Follow infection-control practices, agency policies, and applicable state and federal requirements. Participate in team meetings, care conferences, and training as required. Provide compassionate emotional support to patients and their families. Qualifications Current and active California Licensed Vocational Nurse (LVN) license. Nursing experience required; hospice, home health, palliative care, skilled nursing, or related experience preferred. Strong communication and patient-care skills. Ability to accurately complete clinical documentation. Ability to work independently while following the established plan of care and RN supervision. Valid driver's license and reliable transportation for patient visits. Dependable, compassionate, professional, and committed to quality end-of-life care.

Compensation & Benefits Pay:

Competitive compensation based on experience and employment status. Full-time, part-time, and per diem opportunities may be available.
