The LVN Role:

As a licensed vocational nurse, you will play a crucial role in patient care. Your expertise and understanding of the nursing process will be integral for our agency. You will be required to communicate with our interdisciplinary team and provide professional opinions and create nursing care plans. You will be required to supervise and assess the patients for potential negative outcomes. You will intervene when appropriate and advocate for patients committed to your care. You will complete the required documentation as mandated by Medicare and insurance companies.

Key Responsibilities:

Conduct physical assessments. Perform skills according to your scope of practice. Perform all procedures consistent with "Standards of Care." Complete the mandated documentation. Create/follow comprehensive nursing care plans. Give teaching regarding any, and all, issues that fall under your scope of practice. Promote patient and caregiver independence (when possible). Notify the primary care provider (PCP) of all changes in condition (COC). Maintain patient privacy in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Qualifications:

Current state LVN license. Minimum of 1-year experience in a healthcare or home health setting. Appropriate communication, active listening, critical thinking, and problem-solving skills.
