Deliver compassionate, resident-centered care that promotes comfort, dignity, and safety. Assessment & Monitoring Observe, assess, and document changes in residents' conditions. Report significant findings promptly to the RN Supervisor and attending physicians. Care Planning Assist with implementing individualized care plans. Support residents in achieving rehabilitation goals while maintaining the highest quality of care. Team Collaboration Supervise and support Certified Nursing Assistants (CNAs). Work closely with physicians, therapists, and the interdisciplinary care team to ensure continuity of care. Documentation & Compliance Maintain accurate and timely clinical documentation. Follow all federal, state, and facility policies, procedures, and regulatory requirements.
