BLS Required Position Overview Spanish Peaks Regional Health Center is seeking an experienced Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) for a 13-week travel assignment supporting the Long-Term Care / Skilled Nursing Facility (LTC/SNF) department at the Veterans Community Living Center in Walsenburg, Colorado. The LPN/LVN will provide direct nursing care to residents in a long-term care and skilled nursing environment, working closely with Registered Nurses, physicians, nursing assistants, and other members of the interdisciplinary healthcare team. This position is ideal for an experienced LTC, SNF, nursing home, skilled nursing, or long-term care LPN/LVN who is comfortable working 12-hour night shifts and providing safe, compassionate care to residents with varying levels of medical and nursing needs. Key Responsibilities Provide direct nursing care to residents in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting. Assess residents' conditions and report changes in health status to the appropriate RN or provider. Administer medications and treatments according to physician orders and facility policies. Monitor vital signs, symptoms, pain levels, and overall resident condition. Perform routine nursing assessments and document findings accurately. Assist with the management of chronic conditions and ongoing healthcare needs. Provide wound care and monitor skin integrity as required. Assist with activities of daily living and promote resident safety, comfort, and dignity. Collaborate with RNs, CNAs, physicians, therapists, and other healthcare professionals. Communicate effectively with residents and their families regarding care needs and changes in condition. Maintain accurate and timely nursing documentation. Follow infection-control, medication-safety, and facility protocols. Recognize changes in resident condition and escalate concerns appropriately. Participate in care planning and implementation of individualized resident care plans. Support a safe, respectful, and patient-centered environment throughout the facility. May float to comparable areas based on facility needs. Candidate Requirements Active Colorado LPN/LVN license . Current BLS certification . Previous travel nursing experience . Experience working in a Long-Term Care (LTC), Skilled Nursing Facility (SNF), Nursing Home, or similar setting . Preferred Experience Long-Term Care (LTC) Skilled Nursing Facility (SNF) Veterans or geriatric care Nursing home experience Chronic disease management Medication administration Wound care Post-acute care Rehabilitation / skilled nursing Experience caring for medically complex or elderly residents Work Environment The assignment is located at the Veterans Community Living Center , providing care within a long-term care/skilled nursing environment. The nurse should be comfortable caring for residents with chronic medical conditions, age-related health needs, functional limitations, and other ongoing nursing requirements. The position requires the ability to work nights and maintain a high standard of patient safety, clinical judgment, communication, and resident-centered care.
