We are looking for dependable LPNs who are committed to providing high-quality, person-centered care to our residents in a skilled nursing and rehabilitation setting.

$3,000 Sign-On Bonus Eligible full-time positions qualify for a $3,000 sign-on bonus! $1,500 paid after 6 months $1,500 paid after 1 year LPN Responsibilities Provide safe, compassionate, and high-quality nursing care to residents. Administer medications and treatments as ordered and in accordance with facility policies. Monitor residents' conditions and promptly report changes to the appropriate healthcare provider. Complete accurate and timely nursing documentation and clinical records. Communicate effectively with residents, families, physicians, and members of the interdisciplinary team. Assist with care planning and implementation of individualized resident care plans. Perform assessments and follow up on residents' needs and changes in condition. Supervise and collaborate with CNAs and other nursing team members. Follow infection-control procedures, safety standards, and applicable regulations. Promote residents' dignity, comfort, independence, and overall quality of life. Respond appropriately to emergencies and changing resident needs. Maintain a professional, respectful, and supportive environment for residents and coworkers. Qualifications Current and valid Connecticut LPN license . Previous experience in skilled nursing, long-term care, rehabilitation, or a similar healthcare setting preferred. Strong clinical, communication, organizational, and interpersonal skills.
