WHAT YOU'LL DO

As a hospice licensed practical/vocational nurse (LPN/LVN), you will provide compassionate nursing care that helps patients remain comfortable throughout their end-of-life journey.

You will:

Provide skilled nursing care based on each patient's individualized plan of care Administer medications and treatments and monitor patient response Identify and communicate changes in a patient's condition Educate and support patients and families with medications, medical equipment and care needs Maintain accurate and timely documentation Working alongside your team, you will give your best each day in service of our core commitment: Delivering compassionate, personalized care with heartfelt empathy, honoring the wishes of each patient who entrusts VITAS with their end-of-life journey.

WHERE YOU'LL WORK

Depending on your assignment, you may provide care in patients' homes, assisted living/skilled nursing facilities or other hospice care settings. Some roles may include travel within an assigned service area, on-call responsibilities or after-hours patient and family support.

REQUIREMENTS

Graduate of an approved practical or vocational nursing program Current LPN/LVN license in the state where you will practice At least one year of nursing experience; hospice, home health, hospital or community health experience preferred Reliable transportation, valid driver's license and required auto insurance for roles providing in-person care Additional licensure may be required for certain telephonic or after-hours assignments.
