POSITION SUMMARY

The Licensed Practical Nurse (LPN) - Population Health supports the delivery of proactive, patient-centered care by assisting with care coordination, chronic disease management, preventive health initiatives, and quality improvement activities. Working as part of a multidisciplinary team, the LPN engages patients through outreach, education, and follow-up to improve health outcomes, close care gaps, and promote adherence to treatment plans. The LPN plays a key role in supporting value-based care initiatives, preventive screenings, care transitions, and ongoing management of patients with chronic conditions. HS/GED required Current LPN license in good standing with the Florida Board of Nursing 1+ years Clinical experience in an ambulatory care setting preferred Current CPR certification required Excellent listening and interpersonal skills Must be flexible, resourceful, and able to troubleshoot Must be able to handle multiple tasks simultaneously and set priorities Pride in the job you do and the image you present to our patients & visitors A positive can-do attitude Bilingual (Spanish/English) a plus
