We are looking for nurses who are committed to providing safe, high-quality, patient-centered care while working as part of our growing home health team. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to develop personalized care plans. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Provide wound care, catheterization, tube feeding management, and sterile procedures to support patient recovery and comfort. Monitor patients' conditions closely, recognize signs of deterioration or acute pain, and triage urgent issues effectively. Educate patients and caregivers on disease management, medication administration, mobility assistance, and safety precautions. Maintain accurate medical documentation within electronic health record (EHR) systems while adhering to HIPAA confidentiality policies. Collaborate with interdisciplinary teams including physicians, therapists, and social workers to coordinate comprehensive care. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in home health care or related settings such as skilled nursing facilities or hospitals. Familiarity with EMR/EHR systems like Epic or Cerner for documentation and patient tracking. Strong knowledge of medical terminology, physiology, and clinical assessment techniques. Experience with IV infusion therapy, wound care, catheterization, and medication administration. Ability to perform physical examinations and vital sign assessments accurately. Excellent communication skills for effective caregiver education and client interaction. Knowledge of long-term care regulations, dementia care including Alzheimer's disease management, and infection control protocols.
