St. Francis Hospice (SFH) is growing, and we need a LPN to join our team. SFH is a well-established, mission-driven healthcare organization dedicated to providing exceptional hospice and palliative care services throughout our community. The Licensed Practical Nurse (LPN) is responsible for the accurate assessment of each patient's status, problems and needs. The LPN collects and analyzes data from the patient's medical history, nursing assessments and staff input on an on going basis. The LPN provides PRN visits on patients who call the answering service, crisis care visits as assigned and PRN visits as requested by PCC or MD. Provides care for patients for the entire County. The LPN works collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life. Current and active Florida (LPN) License required. Hospice, home health, palliative care, or related experience preferred.
