Manage IV infusions, catheter care, tube feeding, and ventilator support for patients requiring specialized care such as ICU or long-term care settings. Conduct diagnostic evaluations including specimen collection and processing, phlebotomy, and basic laboratory procedures with attention to infection control standards. Provide patient education on medication administration, pain management strategies, dementia care including Alzheimer's and memory care techniques, and discharge planning. Collaborate effectively with interdisciplinary teams to coordinate case management, triage assessments, and post-acute care planning for diverse patient populations including pediatrics, geriatrics, and individuals with disabilities. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience working in medical-surgical units, emergency medicine, ICU or trauma centers such as Level I or Level II trauma facilities. Demonstrated proficiency in EMR/EHR systems like Point Click Care. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology essential for accurate patient assessment and documentation. Experience delivering specialized care including hospice services, infectious disease management, acute pain control, or geriatrics is highly desirable. Ability to perform first aid procedures and respond swiftly in urgent situations; CPR certification required. Excellent communication skills for caregiver interaction and patient education; experience working with individuals with developmental disabilities or behavioral health needs is advantageous.
