Responsibilities Provide skilled nursing care in accordance with physician orders and the established plan of care. Assess and document each patient's condition, progress, and response to treatment. Administer physician-prescribed medications and treatments. Perform wound care and monitor the healing process. Educate patients and caregivers on medications, treatments, disease management, safety, and home care procedures. Communicate changes in a patient's condition to the supervising RN, physician, and other appropriate team members. Collaborate with physicians, nurses, physical therapists, occupational therapists, and other clinicians involved in the patient's care. Recommend equipment, resources, or strategies that may improve the patient's safety, independence, and quality of life. Complete accurate and timely clinical documentation after each visit. Qualifications Active and unrestricted Florida LPN license. Previous nursing experience; home health experience is preferred. Strong knowledge of nursing practices, infection control, and patient-safety procedures. Compassionate, dependable, and comfortable working independently in patients' homes. Excellent communication, interpersonal, and problem-solving skills. Strong organizational and time-management abilities. Current CPR certification.
