The ideal candidate will possess a strong background in patient care and demonstrate proficiency in various nursing skills. As an LPN, you will play a critical role in providing high-quality care to patients across diverse settings, including hospitals, nursing homes, and assisted living facilities. Responsibilities Provide direct patient care under the supervision of registered nurses and physicians. Administer medications and treatments as prescribed, ensuring compliance with established protocols. Conduct patient assessments, monitor vital signs, and document medical histories accurately. Assist with procedures such as IV infusion, catheterization, and wound care. Collaborate with healthcare teams to develop and implement individualized care plans. Educate patients and their families about health management and disease prevention. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Participate in discharge planning and case management for patients transitioning from acute to post-acute care settings. Qualifications Valid LPN license in the state of practice. Experience in critical care, long-term care, or assisted living preferred. Proficiency with electronic medical records (EMR) systems such as Epic, Cerner, or eClinicalWorks. Knowledge of ICD-9 and ICD-10 coding is advantageous. Familiarity with various nursing specialties including geriatrics, pediatrics, hospice care, and emergency medicine is a plus. Strong skills in patient assessment, medication administration, phlebotomy, and vital signs monitoring. Ability to work effectively with individuals with developmental disabilities or Alzheimer's disease. Excellent communication skills and a compassionate approach to patient care. Basic Life Support (BLS) certification required; additional certifications in specialized areas are beneficial.
