Job Overview We are seeking a motivated and detail-oriented Licensed Practical Nurse (LPN) to join our dynamic home care team. Duties Conduct requires in-home supervisory visits in compliance with state, Medicaid, and agency requirements. Observe and assess caregiver performance and service delivery during supervisory visits. Assess client condition and identify and note any changes or concerns that may affect care. Confirm services align with the approved plan of care. Report visit findings, concerns, or recommendations to clinical leadership as needed. Complete supervisory visit documentation accurately and promptly in the agency's designated system. Ensure documentation complies with regulatory, Medicaid, and agency standards. Identify and report compliance concerns, safety risks, or care delivery issues. Qualifications Active, unencumbered LPN or RN license in the State of Georgia Current Basic Life Support (BLS) certification Valid driver's license with reliable transportation Current TB screening Join our team to make a meaningful difference in the lives of our clients through dedicated nursing care delivered with compassion and professionalism!

$25.00 - $30.00 per hour
