Description of Duties:
The individual in this role will provide high quality direct care services in the medical clinic by doing the following: Collect, Assess and Intervene for Vital Signs, Drug screens, Nursing Assessments, Medication Injections and Observation, Complete and Timely Documentation of All Services and Interventions, Medication Education, Health Maintenance Education (HTN/DM/CHF/Asthma/ETC…), Disease Education and Coping/Management Skills Education, Performing Skills for Client De-escalation, Medication Reconciliation, Obtain Medical Records (from outside sources when needed), Obtain Medication Consents, Maintain Client Privacy and Confidentiality, Obtain Release of Information Consents, Troubleshoot Physician/Provider Difficulties/Problems, Maintain Telemedicine clinic flow, Dispense Medications from MFBHC Pharmacy to Clients at Satellite Clinics, Dispense Prescriptions for Controlled Medications to Clients, Troubleshoot Client/Pharmacy Difficulties/Problems, Check Services Authorizations, Link Clients to Community Resources and Case Management, Collaborate with Medical and Clinical Staff to Provide Quality Client Care, Collaboration with External Providers/Pharmacies/Family to Provide Quality Client Care, Completing Prior Authorizations for Medications, Oversee the Schedules and Workflow in the Clinic to Reduce Client Wait Time and Improve Clinic Efficiency, Transport Medications to Satellite Clinics, Make call backs to client, follow up on medication refills and diagnostic appointment follow up calls. Will serve as a floater between any of the four medical clinics in the event there is a staff storage.
Qualifications:
Vocational/technical program from an accredited college AND Current Georgia license to practice as a licensed practical nurse AND Six months of nursing experience. Valid, unrestricted Georgia Driver's License. Must be computer literate. Agency is an eligible worksite for the VA Education Benefit Program (Veterans) Agency is an eligible work site for the National Health Service Corps and the Public Service Loan Forgiveness Programs Middle Flint Health & Wellness services the following 11 counties: Webster, Marion, Sumter, Schley, Taylor, Crisp, Dooly, Macon, Houston, Peach and Crawford.
