OhioHealth is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered care including medication administration, basic treatments, patient education, and coordination with RNs and providers. Responsibilities Provide safe, compassionate, and culturally sensitive direct nursing care to patients under the supervision of an RN and/or provider. Perform focused nursing assessments, obtain and document vital signs, and monitor patients for changes in condition, reporting concerns promptly. Administer medications and treatments according to Ohio Health policies and scope of practice, including oral, topical, and some parenteral medications as permitted. Assist with wound care, dressing changes, specimen collection, basic respiratory care, and other clinical procedures appropriate to LPN scope. Accurately document all assessments, interventions, patient responses, and communications in the electronic health record in a timely manner. Support patient and family education by reinforcing teaching provided by RNs and providers, ensuring understanding of care plans, medications, and follow-up needs. Assist with admission, transfer, and discharge processes, promoting safe transitions of care and continuity of services. Collaborate with RNs, providers, therapists, and ancillary staff to implement individualized plans of care and participate in interdisciplinary rounds as appropriate. Maintain a clean, organized, and safe patient care environment, adhering to infection prevention, isolation, and safety protocols. Support unit throughput by assisting with patient transport, timely room turnover, and prioritization of patient needs based on acuity. Practice within the legal scope of an LPN in Ohio and in accordance with Ohio Health policies, procedures, and standards of care. Demonstrate commitment to diversity, equity, and inclusion by providing respectful, empathetic care to patients and families from all backgrounds. Participate in ongoing education, skills validation, and competency assessments to maintain and enhance clinical skills. Contribute to quality improvement, patient experience, and safety initiatives through reporting, feedback, and adherence to best practices.
