OhioHealth is seeking a dedicated LPN to join our Healthcare & Medical Services team. Responsibilities Provide direct nursing care to patients under the supervision of an RN or provider, including focused assessments, vital signs, and monitoring of patient status Administer medications and treatments safely and accurately, following Ohio Health policies and state regulations Document all care, interventions, and patient responses in the electronic medical record (EMR) in a timely and accurate manner Assist with procedures, specimen collection, wound care, and basic diagnostic tests as directed Recognize and promptly report changes in patient condition to the RN or provider and participate in updating care plans Educate patients and families on medications, treatments, and self-care, reinforcing teaching provided by RNs and providers Maintain infection prevention and patient safety standards, including equipment cleaning, hand hygiene, and fall prevention practices Collaborate with interdisciplinary team members to support coordinated, patient-centered care across the continuum Support discharge planning by reinforcing instructions and ensuring patients and families understand follow-up care Participate in ongoing education, skills validation, and quality improvement initiatives within the unit and organization Required Skills Medication administration and dosage calculation Basic patient assessment and vital sign monitoring Wound care and dressing changes IV therapy and phlebotomy (where permitted by scope of practice) Electronic Medical Records (EMR) documentation, Epic preferred Infection prevention and control practices Basic life support and emergency response Patient and family education techniques Clinical prioritization and time management Interdisciplinary team communication Understanding of nursing scope of practice and regulatory requirements Safe patient handling and mobility assistance
