Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Assist with medical procedures such as wound suturing, tube feeding, venipuncture, and sterile technique practices. Monitor patients' conditions closely, especially those with acute pain, infectious diseases, or requiring ventilator support; respond promptly to changes. Document all patient interactions thoroughly using electronic health records (EHR/EHR systems), ensuring compliance with HIPAA and confidentiality policies. Collaborate with physicians, case managers, and caregivers to develop and implement individualized care plans for patients with dementia, Alzheimer's disease, or developmental disabilities. Provide education and support to patients and their families regarding medication management, disease processes like geriatrics or pediatrics care, and discharge planning. Support triage activities by assessing urgent needs and coordinating appropriate interventions in emergency or urgent care settings. Maintain proficiency in clinical skills such as catheterization, first aid, CPR/Basic Life Support (BLS), and infection control procedures. Experience Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state regulations. Demonstrated experience working with diverse patient populations including geriatrics, pediatrics, neonatal or neonatal ventilator care. Prior experience in long-term care facilities such as skilled nursing or assisted living communities is highly preferred. Hands-on experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with

ICD-9/ICD-10

coding is advantageous. Background in acute care environments such as hospitals—including ICU or trauma centers—emergency rooms or surgical units is desirable. Knowledge of specialized areas such as hospice care, memory/cognitive disorders (e.g., dementia), infectious disease management or pain control enhances your candidacy. Proven ability to perform clinical procedures including phlebotomy, specimen collection/processing, oxygen therapy (ventilator management), and use of medical imaging support tools when applicable.
