Overview We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team and provide high-quality nursing care to diverse patient populations. The ideal candidate will possess a strong clinical background, excellent communication skills, and a passion for delivering compassionate patient-centered care. Responsibilities Perform comprehensive patient assessments, including vital signs and physical examinations, ensuring accurate documentation within electronic health record (EHR) systems. Administer medications and injections safely and effectively, including IV infusions and catheter care, adhering to sterile techniques and clinical confidentiality policies. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures when necessary. Provide specialized care for patients with complex conditions such as Alzheimer's disease, dementia, or those requiring hospice or palliative care. Support patients with activities of daily living in various settings including long-term care facilities, assisted living communities, and home environments. Collaborate with multidisciplinary teams in acute care settings such as ICU, emergency rooms, or Level I trauma centers to ensure optimal patient outcomes. Educate patients and caregivers on health management strategies including pain management, nutrition, and medication administration. Maintain compliance with all healthcare regulations including HIPAA and long-term care standards while ensuring accurate medical documentation. Experience Valid licensure as a Licensed Practical Nurse (LPN) with current state certification. Demonstrated experience working with geriatric populations in settings such as skilled nursing facilities or assisted living communities. Critical care experience preferred, including ICU or emergency room exposure at Level I or Level II trauma centers. Proficiency in EMR/EHR systems such as Cerner or Athenahealth for documentation and patient management. Experience working with individuals with developmental disabilities or neurological conditions like Alzheimer's disease or memory impairments is highly desirable. Knowledge of
