Job Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where you will play a vital role in delivering compassionate, high-quality patient care across diverse settings. Duties Provide direct patient care by performing physical examinations, vital signs monitoring, and comprehensive assessments. Administer medications, injections, IV infusions, and perform catheterizations following strict safety protocols. Assist with diagnostic evaluations including specimen collection, processing, and basic medical imaging procedures. Manage electronic health records (EHR/EHR systems) accurately while documenting medical records and nursing notes in compliance with HIPAA and confidentiality policies. Support complex care needs such as acute pain management, ventilator management, and tube feeding for patients with diverse conditions including trauma or infectious diseases. Collaborate with interdisciplinary teams to develop and implement individualized care plans, discharge planning, and case management strategies. Conduct patient education on medication administration, health coaching, and post-acute care instructions to promote optimal recovery and well-being. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the state of employment. Proven experience working in various healthcare settings such as hospitals (including ICU or Level I/II trauma centers), skilled nursing facilities, assisted living communities, or home care environments. Demonstrated proficiency with EMR/EHR systems like Cerner or Athenahealth; experience with ICD coding (ICD-9/ICD-10) is highly desirable. Strong knowledge of physiology, anatomy, infection control practices, and medical terminology. Experience working with diverse patient populations including geriatrics, pediatrics (including neonatal ventilator management), and individuals with disabilities or dementia. Skilled in emergency response procedures such as CPR and first aid; experience in triage or urgent/emergency care is a plus. Ability to communicate effectively with patients, families, and multidisciplinary teams while maintaining clinical confidentiality policies.
