Job Overview We are seeking a dedicated and compassionate Long Term Care Licensed Practical Nurse (LPN) to join our healthcare team. Duties Provide comprehensive nursing care to residents in long-term care facilities, including medication administration, wound care, and vital sign monitoring. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident's needs. Conduct thorough patient assessments, document medical information accurately using Electronic Health Records (EHR) systems, and ensure compliance with HIPAA regulations. Assist with activities of daily living such as mobility support, catheterization, tube feeding, and personal hygiene. Manage acute pain and chronic conditions by administering treatments like IV infusions and ensuring proper medication management. Educate residents and their families about health conditions, treatment options, and wellness strategies to promote informed decision-making. Maintain a safe environment by adhering to infection control protocols, aseptic techniques, and long-term care regulations. Provide emergency response when necessary, utilizing skills in airway management, first aid, CPR, and critical care procedures. Requirements Valid LPN license with current state licensure required. Proven experience working in long-term care facilities such as skilled nursing facilities or assisted living communities. Knowledge of EMR/EHR systems like Cerner or Athenahealth for accurate documentation and patient record management. Experience with specialized care areas including Alzheimer's/dementia care, memory care, hospice support, and behavioral health is highly desirable. Familiarity with medical terminology, ICD-9/ICD-10 coding standards, and case management practices. Ability to perform physical examinations, venipuncture, catheterization, and other clinical procedures confidently. Strong communication skills for caregiver communication and client interaction across diverse populations. Competence in managing complex cases involving acute pain management, infectious disease care, trauma (Level I & II trauma center experience), ICU or emergency medicine background is advantageous. Knowledge of long-term care regulations and compliance standards such as NCQA standards is preferred.
