Job Overview We are seeking a dedicated and detail-oriented Medication Coordinator to join our healthcare team. In this vital role, you will oversee the accurate administration, documentation, and management of medications for residents or patients across various care settings. Responsibilities Administer medications accurately according to physician orders, ensuring proper dosage and timing while adhering to HIPAA guidelines to maintain patient confidentiality. Maintain comprehensive medical records and documentation using electronic health record (EHR) systems such as Cerner or similar platforms, ensuring all entries are precise and up-to-date. Monitor residents' vital signs, assess responses to medication, and report any adverse reactions or changes in condition promptly. Assist with specialized care procedures including tube feeding, catheter care, and dementia or Alzheimer's care protocols. Collaborate closely with nursing staff, caregivers, and healthcare providers to coordinate medication schedules and ensure seamless patient care delivery. Comply with all state healthcare regulations and standards set by the State Board of Nursing, ensuring legal and ethical medication practices. Conduct manual handling tasks safely during medication distribution or resident personal care assistance, including heavy lifting when necessary. Educate residents and their families about medication regimens, side effects, and safety precautions to promote understanding and compliance. Experience Prior experience working in assisted living facilities, nursing homes, hospitals, or home care environments is highly preferred. Demonstrated knowledge of medical terminology, physiology, anatomy, and senior or geriatric patient care best practices. Experience working with individuals with developmental disabilities or memory care needs such as dementia or Alzheimer's disease is advantageous. Familiarity with EMR (Electronic Medical Records) systems like Cerner or EHR systems used for medical documentation is essential. Certification in CPR and First Aid is required; additional certifications in hospice care or acute care are a plus. Proven ability to handle medications safely while maintaining strict adherence to HIPAA regulations and patient confidentiality standards. Ability to perform heavy lifting when necessary and work effectively within senior living or community-based care settings.
