Licensed Practical Nurse (Memory Care) As a Memory Care LPN , you play a vital role in delivering compassionate, person-centered care to residents living with Alzheimer's or other forms of dementia. You'll provide clinical support, ensure safe medication management, and foster a calm, engaging environment where every resident feels valued and respected. Key Responsibilities Provide compassionate nursing care to residents within our Memory Care neighborhood Administer medications and treatments in accordance with physician orders and state regulations Conduct health assessments and contribute to individualized care plans Monitor changes in residents' conditions and communicate promptly with leadership and families Support and mentor caregivers to ensure consistent, quality care delivery Maintain accurate documentation and uphold safety and infection control standards Promote a positive, team-oriented culture aligned with Charter Senior Living's mission
Qualifications License:
Active LPN license in the State of Illinois (required)
Experience:
Prior memory care or senior living nursing experience strongly preferred
