Allure of the Quad Cities is seeking a dedicated and detail-oriented MDS Coordinator (RN or LPN) to join our team in Moline, Illinois. At Allure, we are all family. As the MDS Coordinator, you will play a vital role in coordinating resident assessments, developing individualized care plans, and ensuring accurate documentation and regulatory compliance.
Job Responsibilities MDS Assessments:
Complete accurate and timely MDS assessments in accordance with regulatory requirements.
Care Planning:
Coordinate and develop individualized care plans that reflect each resident's needs, preferences, and goals.
Interdisciplinary Collaboration:
Work closely with nursing, therapy, dietary, social services, and other departments to ensure comprehensive resident care.
Communication:
Facilitate communication among residents, families, and the interdisciplinary team.
Documentation & Compliance:
Maintain accurate records and ensure compliance with applicable state and federal regulations.
Quality Improvement:
Participate in quality assurance initiatives to improve resident outcomes and care delivery.
Education & Support:
Provide guidance and education to staff regarding MDS processes, care planning, and documentation requirements. Qualifications Current Illinois RN or LPN license in good standing. Minimum of 1 year of long-term care experience preferred. Previous MDS experience preferred. Current BLS certification required. Strong organizational, communication, and time-management skills.
