LPN - Cardiovascular (Inpatient) LiveWell Homecare Agency Urbana, IL Job Details 1 day ago Qualifications Patient treatment planning BLS Certification Care documentation High school diploma or GED Full Job Description The Registered Nurse (RN) is a professional caregiver responsible for managing a designated group of patients, providing care using the nursing process, technology, and resources within the organization's philosophy.
Key duties include:
Performing unit-specific responsibilities and implementing procedures aligned with practice area standards. Maintaining accurate documentation of nursing care to support team collaboration. Conducting initial patient assessments and developing, implementing, and modifying care plans based on patient and family needs. Ensuring patient and staff safety by reporting hazards and incidents. Communicating effectively with colleagues and offering constructive feedback for professional development. Providing direct care to CVMS patients, including post-op CABG, thoracotomy, and vascular surgery patients. Maintaining a clean work environment and completing departmental competencies. Performing hourly patient rounding and proactively assisting internal and external customers.
Qualifications and skills include:
Current RN licensure in Illinois and certifications in BLS and ACLS (within 1 year). High school diploma or GED. Ability to develop care plans, document accurately, and communicate effectively. Commitment to safety, teamwork, and continuous learning.
