Role:

As an Evening Shift LPN, you will work alongside our dedicated team of healthcare professionals to deliver comprehensive care to our residents and patients.

Your responsibilities will include:

Administering medications and treatments as prescribed by physicians Assisting with activities of daily living (ADLs), such as bathing, dressing, and grooming Monitoring and reporting changes in residents' and patients' conditions Providing emotional support and companionship to residents and patients Collaborating with other healthcare professionals to develop and implement individualized care plans

Responsibilities:

Provide direct patient care to residents and patients, including medication administration, vital sign monitoring, and wound care Develop and maintain accurate and comprehensive records of patient care Participate in interdisciplinary team meetings to discuss patient care and concerns Assist with mealtime, transportation, and other activities as needed Maintain a clean and safe environment for residents and patients

Requirements:

Current LPN licensure in the state of Indiana High school diploma or equivalent required 1+ year of experience in a healthcare setting, preferably in a long-term care or rehabilitation facility Basic Life Support (BLS) certification required
