Job Summary We are seeking a dedicated and professional Licensed Practical Nurse (LPN) to join our team on a full-time basis. The ideal candidate will possess a strong background in patient care, with expertise in various healthcare settings. As an LPN, you will play a vital role in delivering comprehensive nursing services, supporting patient well-being, and ensuring adherence to healthcare standards and regulations. Responsibilities Provide direct patient care by administering medications, injections, and treatments according to physician orders and established protocols. Conduct patient assessments, monitor vital signs, and document medical information accurately within electronic health record (EHR) systems. Assist with wound care, IV infusions, catheterizations, and other clinical procedures while maintaining aseptic techniques. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Educate patients and their families on health management, medication administration, and disease processes such as dementia or Alzheimer's care. Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information. Support discharge planning and coordinate with case management teams for post-acute or long-term care needs. Skills Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation. Critical care experience including ICU or Level I Trauma Center exposure is highly desirable. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Experience working with geriatric populations, including memory care or dementia patients. Skilled in IV infusion, medication administration, venipuncture, and sterile processing techniques. Familiarity with long-term care regulations, skilled nursing facility standards, and hospice or palliative care practices. Excellent patient assessment skills combined with the ability to perform physical examinations confidently.
