Licensed Practical Nurse (LPN) - Part-Time One Home Health | Northwest Indiana About the Role One Home Health is growing, and we're looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our team on a part-time basis. This position provides in-home nursing care to patients throughout Northwest Indiana, with opportunities available in:
Lake County:
Crown Point, Hobart, Gary, Hammond, Dyer, and surrounding communities
Porter County:
Valparaiso, Portage, Porter, Hebron, and surrounding communities If you're passionate about delivering high-quality patient care and enjoy the flexibility and independence of home health nursing, we would love to hear from you. What You'll Do As a Part-Time LPN, you will: Provide professional nursing care under the supervision of a Registered Nurse and in compliance with the Indiana Nurse Practice Act Observe, assess, and promptly communicate changes in patient condition according to agency guidelines Promote a safe and supportive environment for patients, families, and team members Follow established agency policies, procedures, and professional standards Assist with the coordination and delivery of home health services Participate in required meetings and maintain accurate, timely documentation Utilize glucometers and INR monitoring devices as needed Perform basic wound care according to physician orders and individualized care plans Complete other duties and responsibilities as assigned What You'll Bring Required Qualifications Graduate of an accredited nursing program Current and active Indiana Licensed Practical Nurse (LPN) license Minimum of one (1) year of acute care nursing experience or equivalent clinical experience Current CPR certification Valid driver's license and reliable, insured transportation that meets agency requirements Clean driving record Preferred Qualifications Prior home health, community health, or hospice experience Experience using Electronic Medical Records (EMR) systems Comfort with tablet-based documentation and electronic communication Skills & Attributes Excellent verbal and written communication skills Strong interpersonal and relationship-building abilities Ability to interact professionally and compassionately with patients, families, and community partners Effective time management and organizational skills Ability to work independently while remaining an active member of a collaborative care team Why Join One Home Health?
