We are seeking a Licensed Practical Nurse (LPN) to support individuals receiving residential and community-based services. This position is responsible for monitoring client health needs, completing nursing assessments, coordinating medical care, supporting medication management, and providing clinical guidance and training to staff. The LPN works closely with program leadership, direct support staff, families, guardians, pharmacies, and healthcare providers to ensure individuals receive appropriate, timely, and well-coordinated care. Key Responsibilities Monitor and assess the ongoing health and medical needs of individuals served. Complete initial and annual nursing assessments and ensure changes in health status are appropriately addressed. Provide nursing consultation and guidance to program staff regarding client medical needs and treatment plans. Provide first aid, basic medical treatment, and clinical evaluation during urgent or emergent situations. Administer medications as authorized and support proper medication management practices. Maintain accurate and timely clinical documentation, including nursing notes, vital signs, assessments, medical records, and follow-up activities. Develop, implement, and monitor individualized wellness and nursing support plans. Coordinate with physicians, pharmacies, healthcare providers, families, guardians, case managers, and program leadership. Review medical documentation, risk management plans, and health-related records for accuracy and effectiveness. Support clients following hospitalizations, emergency care, and outpatient medical procedures, including appropriate follow-up and transition of care. Serve as a primary nursing contact for medication and pharmacy-related issues within assigned residential programs. Coordinate pharmacy reviews and assist with resolving medication, prescription, and delivery concerns. Provide health, medical, medication, pharmacy, and safety-related training to staff. Participate in client support plan meetings, interdisciplinary team meetings, staff meetings, and other meetings as needed. Assist with health and wellness assessments, risk assessments, and required regulatory documentation. Participate in an on-call rotation and provide guidance regarding urgent medical concerns as needed. Ensure nursing practices and documentation comply with applicable licensing requirements, organizational policies, and regulatory standards. Advocate for the health, safety, dignity, and rights of individuals served. Maintain current knowledge of nursing practices, regulations, healthcare standards, and approaches to supporting individuals with diverse needs. Perform additional nursing and health-related responsibilities as assigned. Qualifications Active Licensed Practical Nurse (LPN) license in the State of Indiana required. High school diploma or equivalent required. Strong clinical assessment, documentation, communication, and organizational skills. Ability to effectively communicate with clients, families, guardians, healthcare providers, pharmacies, and interdisciplinary teams. Comfortable providing staff education and clinical guidance. Ability to respond appropriately to urgent and emergent medical situations. Experience supporting individuals in residential, community-based, behavioral health, developmental disability, long-term care, or similar human services environments is preferred.
