Job Description

Encompass Health Cardinal Hill Rehabilitation Hospital is seeking a dedicated LPN/LVN to join our Healthcare & Medical Services team. In this role, you will provide high-quality, compassionate care to rehabilitation patients, administer medications, monitor patient progress, and collaborate closely with an interdisciplinary team. Responsibilities Provide direct nursing care to rehabilitation patients under the supervision of an RN and in collaboration with the interdisciplinary team Administer medications and treatments safely and accurately according to physician orders and hospital policy Monitor, record, and report patient vital signs, symptoms, and responses to treatment in the electronic medical record Assist with patient mobility, transfers, activities of daily living, and implementation of therapy and rehabilitation plans Perform basic wound care, catheter care, and other routine nursing procedures within licensure scope Recognize and promptly report changes in patient condition, participating in appropriate interventions and escalation Support patient and family education on disease processes, medication regimens, safety, and rehabilitation goals Maintain a clean, safe, and therapeutic environment following infection prevention and patient safety protocols Participate in shift handoffs, interdisciplinary team meetings, and care planning discussions to coordinate care Contribute to a culture of compassion, integrity, and teamwork, supporting continuous quality improvement initiatives Required Skills Medication administration and documentation Vital signs monitoring and patient assessment within LPN/LVN scope Wound care and basic dressing changes Rehabilitation and mobility assistance techniques (transfers, ambulation, use of assistive devices) Catheter care and basic continence management Electronic medical record (EMR) proficiency Infection prevention and control practices Basic Life Support (BLS) and emergency response within scope Patient and family education skills Time management and patient prioritization Interdisciplinary communication and teamwork Understanding of rehab nursing principles and discharge planning support
