Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Assist with diagnostic evaluations such as specimen collection, laboratory processing, and medical imaging coordination. Manage patient triage by evaluating symptoms and determining urgency levels for appropriate care pathways in urgent care and community settings. Provide wound care, catheterization, tube feeding assistance, and ventilator management for patients with complex needs including those in long-term or hospice care. Educate patients and caregivers on health management strategies including medication administration, chronic disease management (e.g., dementia, Alzheimer's), and post-acute care instructions. Maintain accurate medical documentation in compliance with HIPAA regulations and facility policies while ensuring confidentiality of patient information. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience working in urgent care, emergency medicine, hospital settings (including ICU or trauma centers), or long-term care environments such as skilled nursing facilities or assisted living communities. Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; proficiency in
ICD-10/ICD-9
coding is preferred. Demonstrated ability to perform clinical procedures including venipuncture, catheter care, sterile technique, CPR/BLS certification, and basic life support skills. Experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities or dementia (including Alzheimer's and memory care), pediatric patients, and those requiring behavioral health support. Knowledge of infection control practices and long-term care regulations; ability to handle acute pain management and infectious disease cases confidently. Strong communication skills for effective caregiver engagement and client education; ability to work collaboratively within multidisciplinary teams.
