A Licensed Practical Nurse or Vocational Nurse provides basic medical care under the direction of registered nurses and other healthcare specialists. The tasks that they are permitted to perform vary by state, but usually include changing bandages, checking blood pressure, or helping feed patients. May work in hospitals, nursing homes, or other healthcare facilities.

Responsibilities of Treatment Nurse:
1. Reviews communication systems to be knowledgeable of any unusual occurrences during the previous shifts. 2. Gives report to Nursing Assistants regarding skin care needs. 3. Makes rounds on any new admits, hospital returns, or residents with a significant decline to assess the individual resident skin care needs. 4. Performs and/or oversees body audits on all residents on a weekly basis. 5. Prepares and administers wound care treatments as ordered. 6. Provides special procedures as ordered. 7. Assesses resident nutritional needs to aide in wound healing; communicates with physician or delegate and nutritional staff to provide resident with vitamin, protein, nutritional supplements as ordered for wound healing. 8. Assesses and documents, new and old areas of treatment describing in detail the results of treatment and interventions provided. 9. Prepares a weekly skin report for the DON. Calculates weekly wound rate and addresses areas of concern with increase in wound rate noted. 10. Attends High Risk Meeting weekly; reviews and addresses all residents with skin care issues and interventions in place. 11. Contacts the physician for any areas not responding positively and timely. 12. Continually assesses and re assesses residents for risk of complications. (I.e., skin breakdown, incontinence, contractures, reduced mobility, reduced involvement in bathing, dressing, or eating, weight loss, behaviors due to change in lifestyle, etc.). 13. Infection Control Management with residents identified with
MDRO. 14.
Implement skin prevention measures on residents identified at risk for skin breakdown and makes rounds weekly to ensure measures are in place and effective. 15. Assist MDS in performing Skin Risk Assessments on admit, quarterly, annually, with a significant change and as needed. 16. Continually observes progress and quality of work of the Nursing Assistants to assure that residents are turned, repositioned, toileted, walked, exercised, provided food and fluids, well groomed, involved in their specific restorative program, provides intervention as indicated, etc. Also observes the resident's environment to assure a clean and safe environment is provided for each resident. 17. Document on nurse's notes as per policy. Assures there is accurate and timely documentation of any unusual observation, incident, or happening. 18. Reviews information, reassess residents, measure identified skin care issues and documents at a minimum of weekly in the Wound section of the electronic health record. 19. Assists in the ordering and reviewing of results of laboratory tests as indicated. Notifies physician of results. 20. Notifies the physician of unusual findings as indicated. 21. Assists with auditing charts. 22. Assists with answering and correcting consultant reports. 23. Reviews, signs, and provides intervention as indicated for Nursing Assistants documentation. 24. Makes appropriate notes in Communication Books to assure adequate communication is maintained for nursing. 25. Assists in keeping other departments informed of resident's change of condition as indicated. 26. Assists in providing in services for assigned staff. 27. Keeps nurse's station, treatment cart and medication room clean. 28. Reviews and updates Treatment Administration Records. 29. Gives report to nurses and administrative staff to provide continuity of care. 30. Supervises the meal service for each resident on the unit. Works with the staff to assure each resident is provided adequate nutrition for each meal. 31. Performs any other tasks as assigned by the Director of Nursing.

Must be willing to be in the facility ready to begin assigned duties on time. 11. Must be willing to be in the facility during the weekdays as well as weekend hours. 12. Must be willing to work preferred shifts as scheduled and assist in other shifts as needed to assure adequate staffing to meet the needs of each resident. 13. Must demonstrate ability to evaluate the geriatric resident's needs and provide intervention timely to assure each resident's needs are met. 14. Must demonstrate ability to evaluate nursing staff needs and provide staff education timely to provide quality care. 15. Must demonstrate ability to function as a cooperative team member with each member of the Nursing Department and other departments. 16. Must demonstrate the ability to follow and supervise assigned staff to assure the facilities policies and procedures are followed and upheld. 17. Must demonstrate the ability to manage and supervise the skin care program for the facility. 18. Must have an annual Tuberculosis screening exam.
