3005090709-2 Apply for this job now Job Description Brightview Senior Living is seeking a compassionate LPN/RN to provide high-quality clinical care and support to residents in our vibrant senior living community. In this role, you'll deliver medications and treatments, complete assessments, document care, and collaborate with physicians and care teams to support independent living, assisted living, and memory care residents. You'll educate residents and families, help coordinate care plans, and promote safety, dignity, and engagement every day. Responsibilities Administer medications and treatments according to physician orders and community policies Perform resident assessments and monitor changes in condition, reporting concerns promptly Document care accurately in electronic health records (EHR) and maintain regulatory compliance Collaborate with physicians, nurses, and care associates to develop and update care plans Provide resident and family education on health conditions, medications, and care plans Support residents with activities of daily living while promoting dignity and independence Implement and follow infection control, safety, and fall prevention protocols Respond to emergencies and provide BLS/CPR as needed Participate in care conferences, shift reports, and interdisciplinary team meetings Contribute to a positive, resident-first culture and mentor care team members as needed Required Skills Medication administration Vital signs monitoring Wound care Electronic health records (EHR) documentation Care planning Geriatric nursing Infection control Patient/family education Intravenous therapy (IV) basics Fall risk assessment
