This is a full-time, Monday-Friday day-shift position with a flexible schedule. The LPN plays an important role in providing quality, person-centered care and coordinating services for residents in our assisted living community. What You'll Do Assess resident needs and help develop and implement Individual Service Plans (ISPs). Monitor residents' health status and communicate changes to the appropriate healthcare providers. Coordinate care with residents, families, physicians, VNA/home health agencies, PT, OT, and other service providers. Provide guidance and support to Resident Care Assistants and Resident Care Supervisors. Assist with evaluating prospective residents as part of the admission process. Identify safety concerns and initiate appropriate corrective action. Serve as a liaison between residents, families, care staff, administration, and healthcare providers. Promote quality, person-centered care while supporting residents' independence and dignity. Perform other related duties as assigned by the Wellness Director. What We're Looking For Current Massachusetts Licensed Practical Nurse (LPN) license in good standing. Previous experience in assisted living, long-term care, home health, or a related healthcare setting preferred. Strong communication and organizational skills. Ability to work effectively as part of an interdisciplinary care team. Compassionate and resident-focused approach.
