Sign On Responsibilities:

Under the direction and supervision of the Director of Advocacy & Family Support per the AFC Regulations the licensed practical nurses may: Complete and coordinate all applicable clinical assessments and clinical evaluations; Develop and review on an ongoing basis each member's AFC plan of care; Provide oversight to AFC caregivers in conjunction with the care manager; Review the PCP Summary Form; Monitor the health status of all members to ensure that all needed AFC is properly delivered; Report changes in the health status of any member to the member's PCP; Conduct on-site visits with each member at the qualified setting in accordance with 130 CMR 408.415(B)(6); IND123 Licensed Practical Nurse with two years experience with ID/DD populations or the elderly.
