Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to diverse patient populations across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and collaborative care planning. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans. Administer medications and injections accurately following prescribed protocols while monitoring for adverse reactions. Support patients with activities of daily living and provide specialized care such as tube feeding, catheterization, and wound management. Assist with diagnostic evaluations, specimen collection and processing, and basic medical procedures including suturing and sterile techniques. Maintain detailed medical documentation within Electronic Health Record (EHR) systems such as Epic or Cerner in compliance with HIPAA regulations. Collaborate with physicians, registered nurses, case managers, and caregivers to coordinate effective treatment plans. Manage acute pain and infection control measures while ensuring patient safety during procedures like ventilator management or airway support. Educate patients and their families on health conditions, medication regimens, and post-discharge care instructions. Participate in discharge planning, triage assessments, and emergency response protocols including CPR and first aid. Ensure compliance with long-term care regulations and standards such as NCQA standards for quality assurance. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). Proven experience in diverse clinical settings such as hospitals (including ICU or Level I trauma centers), nursing homes, hospice care, or outpatient clinics. Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth; proficiency in

ICD-9/10

coding is preferred. Demonstrated skills in medication administration, IV infusion therapy, venipuncture, phlebotomy, and catheter care. Knowledge of physiology, anatomy, pharmacology principles, and medical terminology relevant to patient assessment. Experience working with geriatric populations or individuals with developmental disabilities is highly desirable. Ability to perform physical examinations accurately and efficiently while maintaining clinical confidentiality policies. Strong communication skills for effective caregiver interaction and client education. Certification in Basic Life Support (BLS) required; additional certifications such as ACLS or PALS are advantageous. Flexibility to work varied shifts across inpatient or outpatient settings; travel nursing experience is a plus.
