Per Diem About the Role Are you an LPN who loves working directly with people in the community? Do you enjoy combining your clinical skills with teaching and empowering others? If so, we'd love to meet you! We're looking for a compassionate and self-motivated Licensed Practical Nurse (LPN) to join our team in supporting members of the Massachusetts Personal Care Attendant (PCA) Program . In this role, you won't just be doing assessments—you'll be a mentor, educator, and advocate, helping members gain the skills and confidence they need to live independently and manage their care safely.

What You'll Be Doing Assessments & Evaluations:

Visit members in their homes or community settings to assess their functional needs (ADLs/IADLs) and determine the appropriate level of PCA support.

Hands-on Coaching & Training:

Teach and coach members and their surrogates on how to recruit, hire, train, and manage their own PCAs effectively.

Service Planning:

Collaborate with members to build and maintain accurate Service Agreements and handle necessary MassHealth/PCM documentation.

Team Collaboration:

Work closely with our internal team (Occupational Therapists, case managers, and administrative staff) to ensure high-quality care and support.

Flexible Scheduling:

Manage a field-based schedule with a blend of home visits and remote/office administrative time.

What We're Looking For License:

Current, Massachusetts LPN license .

Experience:

Background in home care, disability services, community health, or rehabilitation.

Skills:

Strong clinical assessment skills combined with a passion for teaching, coaching, and empowering others.

Tech & Organization:

Good documentation habits and familiarity with program guidelines.

Travel:

A valid driver's license and reliable transportation for field visits in the community.
