Role Overview The Wellness Nurse LPN is responsible for monitoring resident health, supporting medication management, and ensuring clinical quality within a senior living community. Key duties include conducting assessments at move-in, routine wellness visits, and during change-of-condition events; monitoring health trends; following up on incidents such as falls and hospital returns; transcribing physician orders; completing medication audits; and maintaining compliance with infection control and regulatory standards. Qualifications & Skills:
