LPN LONG TERM CARE

Hours Days Shift Details:
5.30 AM to 6.00 PM 36 hours/week. (3
  • 12) 13-week contract Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories, to develop individualized care plans.
Administer medications, including injections and IV infusions, following prescribed protocols and ensuring accurate documentation within electronic health record (EHR) systems. Monitor residents for signs of acute or chronic health issues such as pain management needs, infections, or dementia-related behaviors. Provide wound care, catheterization, tube feeding, and other specialized procedures with strict adherence to aseptic techniques. Collaborate with interdisciplinary teams to coordinate care plans, discharge planning, and case management activities tailored to each resident's needs. Educate residents and their families on health conditions, medication regimens, and safety protocols while maintaining compliance with HIPAA confidentiality policies. Respond promptly to emergencies or urgent situations using first aid skills and airway management techniques; participate in triage assessments as needed. Maintain accurate medical documentation in EMR/EHR systems and ensure compliance with long-term care regulations and standards. Requirements Valid Licensed Practical Nurse (LPN) license with current registration in the applicable jurisdiction. Prior experience working in long-term care facilities such as skilled nursing facilities or assisted living communities is highly preferred. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation purposes. Knowledge of medical terminology, anatomy, physiology, and ICD coding for accurate recordkeeping and billing processes. Experience administering IV infusions, injections, catheter care, and wound management procedures. Strong patient assessment skills coupled with the ability to recognize signs of acute or chronic health deterioration. Excellent communication skills for effective caregiver communication and collaboration within multidisciplinary teams. Ability to work independently while adhering to safety protocols including HIPAA regulations and infection control policies. Additional experience in geriatrics, dementia or Alzheimer's care, hospice support, or behavioral health is advantageous.
